I’m a TCU employee. How do I report a positive COVID-19 case for myself or a direct report?

Employees who test positive for COVID-19 or exposed to COVID-19 must report this information to campus by completing a TCU Self-Report Form.
Employees may also email email@example.com or call TCU’s COVID-19 Self-Reporting Hotline at 817-257-2684 if they have questions. Once submitted, a TCU representative from Human Resources, contact tracing, and/or TCU Facilities will follow-up.
Supervisors also may use the email or hotline number to report an employee who has a confirmed positive test. Supervisors may report a positive case or an exposure on behalf of employees – IF the employee is not able to self-report.
